Evaluation de la Douleur et de L’anxiété Liées à la Chirurgie de la Cataracte

Objectifs : Répertorier et analyser les facteurs liés à l’anxiété et à la douleur liées à la chirurgie de cataracte sous anesthésie locorégionale à Lomé. Matériels et méthodes : Nous avons réalisé une étude transversale, sur une période de 7 mois de Janvier à Juillet 2020, sur des patients devant bénéficier d’une chirurgie de la cataracte. L’anxiété préopératoire a été évaluée à l’aide de l’Amsterdm preoperative anxiety and information scale (APAIS cotée de 6 à 30) avant chaque intervention. La douleur postopératoire était mesurée par l’échelle visuelle analogique (EVA coté de 0 à 10). Résultats : Au total 105 patients ont été inclus, dont 50 hommes et 55 femmes soit un sex-ratio (H/F) de 0,91. L’âge moyen était de 61,71±14,14 [22-88] ans. La durée moyenne de la chirurgie était 22,52±6,73 minutes. Le score moyen d’anxiété préopératoire globale était de 7,22 ± 3,14. Les femmes avaient un score moyen d’anxiété globale de 8,04 contre 6,32 pour les hommes. Le score moyen d’anxiété préopératoire globale du premier œil était de 6,75 contre 8,72 pour le deuxième œil. Le facteur préopératoire le plus anxiogène était : le sentiment de ressentir une douleur postopératoire (19,05 %) suivi de la possibilité de perdre l’œil opéré (9,52 %). Parmi les 105 patients, 5 (4,76%) ont déclaré une douleur postopératoire immédiate égale à 4 selon l’EVA pour le premier œil contre 8 (10,26%) pour le deuxième œil. En postopératoire immédiat, 12,73% des femmes ont exprimé une douleur à 4 selon l’EVA contre 12% d’hommes. Conclusion : La gestion de l’anxiété et de la douleur liées à la chirurgie de la cataracte passe par l’information du patient sur l’anesthésie, la chirurgie, le suivi post opératoire, et sur le résultat visuel attendu.
Summary
Aims: To identify and analyze factors related to anxiety and pain related to cataract surgery under loco-regional anesthesia in Lomé. Methods: We conducted a cross-sectional study, over a period of 7 months from January to July 2020, including patient undergoing cataract surgery. Preoperative anxiety was assessed using the Amsterdam preoperative anxiety and information scale (APAIS rated from 6 to 30), prior to each intervention. Post-operative pain was measured by the analogic visual scale (AVS rated from 0 to 10). Results: A total of 105 patients were included, including 50 men and 55 women with a sex ratio (H/F) of 0.91. The average age was 61.71±14.14 [22-88] years. The average duration of surgery was 22.52±6.73 minutes. The average overall preoperative anxiety score was 7.22 ± 3.14. Women had an overall average anxiety score of 8.04 versus 6.32 for men.
The average overall preoperative anxiety score for the first eye was 6.75 versus 8.72 for the second eye. The most anxiety causing preoperative factor was: the fear of postoperative pain (19.05%) followed by the possibility of losing the operated eye (9.52%). Over the 105 patients, 13 (12.4%) reported immediate postoperative pain of 4 on the analogic visual scale AVS versus 8 (10.26%) during the second eye procedure. In immediate postoperative, 12.73% of women expressed pain at 4 on the AVS compared to 12% of men. Conclusion: Management of anxiety and pain related to cataract surgery involves information of patients about the anesthesia, the surgery, the postoperative follow-up and the expected visual result
